Care Navigation Sessions Now Available through the PAA!

Have you been newly diagnosed with Parkinson’s? Or, perhaps you have been diagnosed for years and need a boost in understanding what new resources may be available to you? The Parkinson Association of Alabama (PAA) is pleased to provide personalized Care Navigation sessions to those diagnosed with Parkinson’s in Alabama and their care partners.



By diving deeply into your unique journey, we can help you craft a packet of resources tailored to meet your needs and give you a great start to managing life with Parkinson’s well.


What is a Care Navigation Session? We will schedule a zoom meeting (preferred) or a phone call with you between the hours of 9am and 2pm, Monday – Thursday. We strongly encourage your care partners to attend if available. Any other family members or friends you would like to join are welcome as well. We will commit up to 90 minutes for the sessions. From there, we will walk you through an assessment that will allow you to:

  • Ask any questions you may have about Parkinson’s while providing an overview of PD

  • Consider what boxes to check when it comes to future planning and long-term care plans

  • Talk about preparations for home and hospital safety

  • Provide tips for staying organized and optimizing time with your specialists

  • Plug you into your local programs and resources based on your interests – including support and movement groups – as well as other Parkinson’s specific programs in your area

Through this assessment, we will cultivate the resources specifically tailored to your needs and present them back to you with a newly diagnosed kit and the opportunity to connect to your local resources. We can provide the newly diagnosed kit digitally through email (preferred) or mail.


Confidentiality Promised The information you share with us through your assessment is confidential and protected through HIPPA. We will not share, save, store, or disclose any information you provide to us. Our only goal is to best understand your journey to plug you into the best resources available to you.


Who are Care Navigators? Currently, the PAA has one Care Navigator on staff. As demand increases and funding becomes available through generous donations, the hope is to expand to three care navigators throughout the state of Alabama: Northern, Central and Southern regions.

Care Navigators can be nurses, social workers, or highly-trained professionals that have lived the Parkinson’s journey and know the best resources to provide. The PAA will ensure that all Care Navigators are trained to offer a unified, compassionate, and helpful experience worthy of the PAA Care Navigation Assessment program.


How do you schedule a care navigation session with the PAA?
